Complaint against Xercise4Less

After 12 months membership and due to lower back problems i decided to take a break from Xcerise for Less in Derby. I didn't originally complete the online cancellation as I have terrible internet access at home, so I handed in a written notice to the receptionist (approx July/Aug2017), a month later I cancelled my direct debit. for the past few months I have being contacted via Harlands despite contacting both them and the gym manager and informing them of the situation. the manager advised that I went on line and complete the cancellation form again (which I did) but Im still being told Im in default and need to pay. Any advise would be welcome I already suffer from anxiety and this doesn't help. Many thanks
